Case No. 3AN-17-01829 PR
NOTICE OF FILING PETITION FOR ADOPTION AND HEARING

TO: NANCY A. HENSLEY
You are hereby notified that on July 11, 2017, the petitioners, Lisa Marie Gonzalez and Joaquin Gonzalez, filed in this Court a Petition for Adoption of Alexis Ann Hensley, a minor, whose date of birth is April 21, 2004, and for change of the minors name to Alexis Ann Gonzalez. This Court, located at 825 West 4th Avenue, Anchorage, Alaska 99501, Courtroom 203, will hear the petition on January 2, 2018, at 3:30 pm.
It is alleged in the petition, pursuant to AS 25.23.050, that your consent to the adoption is not required due to the following:
1. That, without justifiable cause, you abandoned the child for a period of at least six months;
2. That, for a period of at least one year, you have failed significantly without justifiable cause, including but not limited to indigency,
(A) to communicate meaningfully with the child, or
(B) to provide for the care and support of the child as required by law or judicial decree;
You are further hereby notified that a final decree of adoption, if granted, will relieve you of all parental rights and responsibilities, including the right to contact the minor, and, except with respect to a spouse of the adoption petitioner and relatives of that spouse, terminate all legal relationships between the minor and you and the minors other relatives, so that the minor will thereafter be a stranger to you and the minors former relatives for all purposes. If you wish to contest the adoption, you must notify the court in writing and also appear at the hearing on the petition now scheduled for January 2, 2018 at 3:30 pm at the Nesbett Courthouse, 825 West 4th Avenue, Anchorage, AK 99501, Courtroom 203. A final decree of adoption may be entered if you fail to file an objection to the adoption petition or appear at the hearing.
